In order to ensure proper treatment, it is necessary for the treating therapist to build up a file. This is also a legal obligation imposed by the WGBO. The file contains notes on your state of health and data on the examinations and treatments carried out.<\/p>\n

Life Force Neuromuscular Therapy does its utmost to guarantee your privacy. This means, among other things:<\/p>\n

    \n
  • careful handling of your personal and medical data,<\/li>\n
  • ensure that unauthorised persons cannot access the data<\/li>\n<\/ul>\n

    The therapists at Life Force Neuromuscular Therapy have a legal duty of confidentiality (professional secrecy).<\/p>\n

    The data from your file can be used for the following purposes:<\/p>\n

    • To inform other healthcare providers, for example when the therapy is completed or in the event of a referral to another practitioner . This only happens with your explicit permission.<\/li>\n
    • For use in observation, during absence.<\/li>\n
    • For anonymised use during peer review.<\/li>\n
    • A small part of the data from your file is used for financial administration so that we can prepare an invoice.<\/li>\n<\/ul>\n

      If we want to use your data for any other reason, we will first inform you and ask for your explicit permission.<\/p>\n

      The data in the client file will be kept for 20 years as required by the Treatment Agreement Act.<\/p>\n

      Privacy on the healthcare bill<\/h3>\n

      The care invoice you receive will contain the information required by the health insurance company, so that you can claim this invoice from your health insurance company.<\/p>\n

      • your date of birth<\/li>\n<\/ul>\n

        Rights regarding your data<\/h3>\n

        You have the right to inspect your file, within the walls of the practice. You also have the right to rectification or removal of the personal data we have received from you.<\/p>\n

        Complaints<\/h3>\n

        If you have a complaint about the processing of your personal data, please contact us immediately. If we cannot find a solution together, you always have the right to submit a complaint to the Netherlands Authority for the Protection of Privacy.<\/p>\n
